I am on the title of our home but want to add my husband. Do i need to add him to the title or the deed?

When we refinanced, we said we only have the title so we could get the best rates. They told me that I could add more? later. This would add to the title or legal document? I felt that to add it to the title I had to refinance and add to the legal document would have been more? easy. I don 't know the difference and want to make sure that if something happens I do not lose any interest in the house.
